1. A Medication Permission Form must be completed and signed.
2. A parent or guardian must bring the medicine to the school office.
(Medicines must not be transported with students on the bus.)
3. Over-the-counter medication must be brought to school in the original container,
with the seal intact.
4. Prescription medication must be in the original container. The student’s name, medicine name, and directions must be clear.
